Its happening again. I lost the quest and stopped receiving insignias. refreshed, cleared cache and tried different browsers

I had same thing, but it worked the next time. It was fine on main account, but not on any alts. (was this on your 1st toon or alt / 2nd 3rd etc) I used the bug feature also.
